Okay, so it's the general of his army. And then it makes me wonder how the mothers fit in. Like did all the mothers know the YED or was Mary special somehow?

Dean is sacrificing his fantasy/dream self for Sam mostly, is how I took it. Yes, saving other people as well is important to him, but I took the fact that the last person he looked at before stabbing himself being Sam was that he was going back to protect his Sam, that he would sacrifice everything for that cause. So now I'm wondering if that's going to play into part two of the finale.
